• Traveler Policy: Applicants within 50 miles of facility are not eligible. Applicants who were direct employees within the last 2 years are not eligible. Candidates Permanent Address (Facility will not consider local candidates - 50-mile radius restriction)
  

Key Responsibilities:

• Apply knowledge of anatomy exercise physiology biomechanics kinesiology neuroscience and pharmacology.
  
• Assist other facility staff members as needed to provide care and create a clean orderly and effective environment for rehabilitation and physical therapy.
  
• Develop a plan for patients following completion of their Plan of Care with an appropriate home exercise program and discharge instructions.
  
• Develop a Plan of Care for each patient based on initial and ongoing evaluations including short and long-term goals collaboration demands of other staff members and required patient and caregiver involvement.
  
• Document patient progress according to approved medical documentation and facility documentation procedures.
  
• Evaluate mobility strength and current level of patient function while setting goals to improve in these areas that are scaled to patient abilities and realistically attainable.
  
• Implement the Plan of Care on a consistent schedule and adjust this schedule and plan according to patient progress.
  
• Maintain open lines of effective communication between therapy staff department staff and referral sources involved in the care of the patient.
  
• Maintain patient records ensuring that Certifications and Authorizations are in compliance with third party payer requirements.
  
• Maintain professional and technical knowledge through attending educational workshops; reviewing professional publications etc.
  
• Perform initial and ongoing physical therapy evaluations according to APTA standards of practice as well as state standards.
  
• Strong experience administering a wide range of physical therapy techniques.
  
• Supervise physical therapy assistants as indicated in a professional and consistent manner.
  

Compensation:

• Weekly Pay (hourly) Non-taxed Stipend (if eligible)

• Must have 1 years of PT Experience
  
• Must have outpatient Experience
  
• Must have Active Illinois/IL Physical Therapist License
  
• Must have current Basic Life Support (BLS) certification by the American Heart Association
  
• Excellent communication skills. Ability to work independently.
