POSITION SUMMARYThe Lifespan Psychiatric APRN provides comprehensive psychiatric evaluation medication management and treatment planning for children adolescents and adults. This role collaborates with an integrated care team to support whole-person health and improve outcomes across the lifespan. The provider will function as a member of a multi-disciplinary team and may provide services within one or more Wheeler programs and the relative concentration between programs may change with the needs of the staff on-site include: Primary Care providers Medical Assistants Care Management staff front desk staff and behavioral health Pharmacy and Quest Laboratory on full-time salary of $120000-$142000 based on years of experience.$10000 sign-on bonus availableLOCATION Federally Qualified Health Center (FQHC) located in Bristol CT. Triages clinical emergencies and implements program specific safety and consultation protocols. Utilizes best practices and evidenced based practice models where appropriate and adheres to model and program knowledge of internal and external levels of care referral processes and resources. Makes clinically appropriate referrals for identified service needs in a timely in a positive effective manner with client family and all internal and external providers. Attends any required meetings and direct service productivity goals as defined by program. Demonstrates knowledge of program/department/agency goals and target outcomes and adheres to practice standards and to data collection and reporting all client related information in compliance with agency and is prepared for scheduled supervision.
