Psychiatric Mental Health Nurse Practitioner Geriatrics

Senior Doc part of Brigade Health & Affiliates is hiring a mission-driven Psychiatric Mental Health Nurse Practitioner (PMHNP) to provide high-quality geriatric psychiatry and behavioral health services in home assisted living memory care and skilled nursing settings.

• Integrated continuum-based behavioral health model

• Focus on dementia-related behaviors depression anxiety psychosis mood disorders and complex psychiatric comorbidities

• Work within a collaborative interdisciplinary team

• Autonomy with strong operational and clinical support

• Clear pathway into clinical leadership and program growth for high-performing providers

• Comprehensive psychiatric evaluations & medication management

• Management of behavioral symptoms in dementia and cognitive decline

• Collaboration with PCPs palliative care and facility partners

• Caregiver education and relationship-based care

• Documentation care coordination and panel management

• Active PMHNP-BC license

• Experience in geriatric psychiatry SNF ALF home-based care or community psychiatry

• Strong diagnostic and medication management skills

• Ability to practice independently while contributing to a team-based model

• Leadership potential with interest in growing a successful patient panel
