Position Summary
- Oversee crisis response and high-acuity service of operations including clinical decision making risk mitigation safety practices and continuity of care.
- Develop implement and monitor treatment plans for everyone on caseload; coordinates and consults with others to develop treatment plans.
- Complete diagnostic assessments and provide individudal therapy in correctional settings and office-based setting.
- Provide clinical supervision to reentry staff (Case Manager).
- Coordinate referrals and care plannig while supporting communication among courts and community partners.
- Develop and maintain pre-release plans in collaboration with clients jail staff courts probation treatment providers and other partners.
What We Value in our next Program Coordinator - Reentry Support Services
- Masters Degree in social work counseling or other related field.
- Independent Ohio license in either social work or counseling.
- Experience working with individuals involved in the legal system and working on a multi-disciplinary team.
- Knowledge of co-occurring mental health and substance use disorders.
