To this end in partnership with the New York City Department of Health and Mental Hygiene (DOHMH) FPHNYC incubates innovative public health initiatives implemented by DOHMH to advance community health throughout the city. Itfacilitates partnerships often new and unconventional between government and the private sector to develop test and launch new initiatives. These collaborations speed the execution of demonstration projects effect expansion of successful pilot programs and support rapid implementation to meet the public health needs of individuals families and communities across New York Culture:We embrace a culture of learning collaboration innovation and well-being where open communication drives our the Program:The program focuses on integrated school- and community-based strategies that advance chronic disease prevention physical activity nutrition access and health equity. This position will report to the Sr. Director of Engagement & Capacity The Role:The Program and Data Manager is the senior programmatic and operational lead for the Harlem Bureau of Neighborhood Healths Creating Healthy Schools & Communities program a New York State Department of Health funded Program and Data Manager plays a critical role in operationalizing grant goals into sustainable community-centered activities aligned with the Harlem BNHs priorities cross-bureau initiatives and community residents and : Systems Building & Data Management:Oversee data collection monitoring and reporting for programmatic community engagement and evaluation activitiesIdentify and disseminate best practicesUtilize Public Health Partners Connect and other Harlem Bureau data systems to track participation partnerships and CHSC data with broader Harlem Bureau reporting systemsPrepare reports dashboards and presentations for internal teams funders and external partnersPresenting neighborhood health data to partners to inform shared strategies and collective actionOther duties as assigned are to support the overall goals and mission of the department/ & School Partnership DevelopmentDevelop and maintain strong relationships with schools community-based organizations city and state agencies and local community and support coalitions to leverage resources strengthen coordination and increase program sustainabilityProvide training and technical assistance to partners implementing CHSC-aligned activitiesRequirements: Masters degree in Public health2-3 years of experience managing complex community-based public health programs2-3 years of experience with grant management budgeting and contract oversight2-3 years of experience with schools and community-based partnersPreferred Skills: Strong background in health equity chronic disease prevention and community wellnessStrong data analytics reporting and presentation skillsActive collaborator with varied audiencesExcellent written verbal and interpersonal communication skillsStrong coordination and organizational skills and ability to handle multiple prioritiesDetail oriented; able to manage projects and work independentlyGood time management for planning and decision makingPositive attitude strong work ethic and excellent customer-service skillsProficient with Microsoft Office suite including Word Excel and is contingent upon the successful completion of a background :Public Service Loan Forgiveness (PSLF) eligible employerGenerous Paid Time Off (PTO) policyMedical dental and life insurance with low or no employee contributionA retirement savings plan with generous employer contributionFlexible spending medical and commuter benefits planMeaningful work at an organization striving to advance health equity and social Employment Opportunity Statement:FPHNY is an equal opportunity employer and prohibits discrimination and harassment of any kind.
