Job Title: Home Health - PRN - Physical Therapy Assistant (PTA)
Location: Uvalde TX 78801
Agency: Tri County Home Health Care
Job Responsibilities:
- Provide physical therapy services to patients in their homes.
- Assist the physical therapist in developing and implementing treatment plans.
- Monitor and document patient progress and adjust treatment plans as needed.
- Communicate with physicians nurses and other healthcare professionals regarding patient care.
- Provide education to patients and their families regarding home exercise programs and other aspects of their care.
- Ensure compliance with all regulatory and accreditation standards.
- Maintain accurate and complete patient records.
- Participate in team meetings and case conferences.
- Other duties as assigned.
Skills Required:
- Home Health - Physical Therapy Assistant (PTA) certification/license.
- Experience in providing physical therapy services in a home health setting.
- Ability to work independently and as part of a team.
- Excellent communication and interpersonal skills.
- Strong organizational and time management skills.
- Valid drivers license and reliable transportation.
- Home Care Home Base experience (Preferred)
