As a Home Health Physical Therapist youll help patients regain strength restore mobility improve balance and rebuild confidenceall while providing care where they feel most comfortable.
Youll have the time to focus on each patient create personalized treatment plans and celebrate the progress made one visit at a time.

- Complete evaluations reassessments and discharge visits
- Develop individualized plans of care based on patient goals
- Provide skilled physical therapy services in patients homes
- Educate patients and caregivers on mobility therapeutic exercises fall prevention and home safety
- Monitor patient progress and modify treatment plans as appropriate
- Collaborate with physicians and interdisciplinary team members
- Complete timely and accurate clinical documentation
- Active Virginia Physical Therapist License Required
- Valid Drivers License and Reliable Transportation
- Strong Clinical Assessment and Documentation Skills
- Excellent Communication and Organizational Skills
- Home Health Experience Preferred but Not Required
- Experience with OASIS is a plus
