As a Home Health Physical Therapist youll help patients recover from illness injury surgery and hospitalization while improving strength balance mobility and overall function.

• Complete evaluations reassessments and discharge visits
• Develop individualized treatment plans based on patient goals and needs
• Provide skilled physical therapy services in patients homes
• Educate patients and caregivers on exercise programs mobility training fall prevention and safety strategies
• Monitor patient progress and adjust treatment plans as needed
• Collaborate with physicians and interdisciplinary team members
• Maintain timely and accurate clinical documentation

• Active Pennsylvania Physical Therapist License Required
• Valid Drivers License and Reliable Transportation
• Strong Clinical Communication and Documentation Skills
• Home Health Experience Preferred but Not Required
