In this role youll make a meaningful impact by:

• Provide home-based care to patients with illnesses injuries and chronic diseasesoften after discharge from a hospital rehab or nursing facility
• Work with a team of physicians and oversee LPNs CNAs and aides providing patient care
• Examine patients do assessments document progress and report changes to their physician
• Educate patients on their plan of care prescribed medication therapy diet and exercise
• Coach family members on the patients plan of care medication and home safety
• Ensure goals are met and coordinate discharge from services

A few must-haves for Home Health Registered Nurses:

• 1 year of home healthcare experience
• Oasis experience required
• Graduate of an accredited nursing program and active RN license in FL
• Must maintain current CPR certification a valid drivers license reliable transportation and current auto insurance.
• Strong clinical nursing expertise with a thorough understanding of state and federal home health regulations complemented by excellent communication skills and a compassionate patient-centered approach.
