Job Description:

Bring skilled compassionate nursing care directly to patients in their homes. As a PRN Home Health LPN with Elara Caring youll enjoy flexibility one-on-one patient care and the support of an experienced clinical team.

• Provide skilled nursing care under the direction of an RN and physician

• Follow individualized plans of care and physician orders

• Monitor patients and report changes to the RN Case Manager

• Educate patients and caregivers on medications treatments and self-care

• Complete accurate and timely clinical documentation

• Collaborate with the care team to improve outcomes and help prevent avoidable hospital visits

• Current Massachusetts LPN license

• Graduate of an accredited nursing program

• At least one year of home care experience

• Valid drivers license reliable transportation and current auto insurance

• Ability to travel throughout Malden Lowell and surrounding communities

• Home health or hospice experience preferred

• Ability to sit stand bend lift and move intermittently and lift 50100 lbs.
