Position Overview:

Qualifications

• Graduate of an accredited nursing program
• Active nursing license in the applicable state
• New graduates are welcome comprehensive training is provided!

Responsibilities

• Provide skilled nursing care according to the care plan
• Conduct evaluations including monitoring medications and vital signs
• Administer prescribed medications
• Educate clients and families on home health care best practices
• Coordinate with physicians physical therapists and the care team
• Recommend tools or devices to enhance patient quality of life
