Job Summary
- Perform comprehensive in-person assessments for senior participants.
- Lead and partner with the interdisciplinary care team to develop individualized care plans.
- Provide thoughtful primary care with an average of approximately eight patients per day.
- Coordinate care transitions including acute short-term and long-term placements.
- Partner with Nurse Practitioners Physician Assistants nursing social work therapy and other care team members.
- Participate in regular care planning discussions focused on whole-person health.
- Support home-based care needs through scheduled home visits as part of the role.
- Participate in a rotating phone-only call schedule a few nights or weekends per month.
- Physician trained in Internal Medicine Family Medicine Emergency Medicine or Geriatrics.
- Successful completion of an ACGME-accredited residency program.
- Active state physician license without restrictions.
- State CDS certification and DEA registration.
- Eligible to provide services to Medicare and Medi-Cal participants.
- Board certification preferred.
- CPR and First Aid certification preferred upon hire or ability to obtain within required timeframe.
- Experience in geriatric care senior care value-based care or complex primary care strongly preferred.
- Experience working in an interdisciplinary team model preferred.
- Experience leading or collaborating closely with Nurse Practitioners or Physician Assistants preferred.

Required Skills:
