Unlike traditional primary care settings our value-based PACE program empowers you to provide comprehensive integrated care focused on quality not volume. With a smaller patient panel youll be able to dedicate the time and attention each participant deserves.
What Youll Do
- Conduct thorough in-person physical and wellness assessments.
- Collaborate with the Interdisciplinary Team to design and implement individualized care plans.
- Coordinate participant transitions between acute short-term and long-term care settings.
- Focus exclusively on holistic care no hospital rounds with an average of eight patients daily.
- Completion of an accredited residency program (ACGME required)
- MD or DO degree
- Board Certification preferred
- Active unrestricted California state medical license
- State CDS certification and DEA registration
- Eligibility to participate in Medicare and Medi-Cal programs
- Specialties considered: Internal Medicine Family Medicine Geriatrics and Emergency Medicine
