Job Description:

Looking for a nursing role that keeps you involved in patient care without working bedside shifts

As a Pre-Admission Coordinator youll play a key role behind the scenes by reviewing referrals coordinating admissions and helping patients transition smoothly into home health services. This is a great opportunity for an LPN who enjoys organization communication and being part of a collaborative clinical team.

Review patient records prior to admission
Ensure clinical documentation is complete and accurate
Coordinate with referral sources facilities and clinical teams
Track pending admissions and discharge timelines
Support smooth transitions into home health services
Communicate with patients families and healthcare partners
Utilize systems such as Homecare Homebase and Workday

Active LPN/LVN license
1 year of healthcare experience
Home Health or Hospice experience preferred
Strong communication and organizational skills
Ability to multitask and problem solve in a fast-paced environment
Basic Microsoft Office skills and ability to learn EMR systems
Bilingual English/Spanish preferred
Valid drivers license and reliable transportation
