Harris County Resources for Children and Adults was founded in 1966 as the Harris County Child Welfare Board to provide support services for the children taken under the wing of the Texas Department of Family and Protective Services. Fifty years later Harris County Resources for Children and Adults still provides this support. But the now-county department has expanded services for early prevention intervention and adult services.

Post Transition Service Supervisor Overview:
Under the supervision of the Manager of Transitional Services this position will be responsible for ensuring program compliance with the terms of the Preparation for Adult Living (PAL) contract Council on Accreditation (COA) requirements and internal Quality Improvement Program requests.
Duties & Responsibilities:
Supervisory responsibility for 5-7 HAY Center case manager and coordinator staff
Ensures that staff achieves goals/objectives of the HAY Center and fulfills PAL contractual obligations by following all program policies/procedures with a concentration on but not limited to post-transition services for current and former foster youth preparing to transition into adulthood.
Manages retrieval of internal and external referrals and eligibility documentation assigns/monitors cases and maintains EVOLV (case management) database and PAL (contract reporting) database for post-transition services.
Completes all case management weekly/monthly reports as required by the PAL contract and submits to direct supervisor.
Reviews all case management requests for payment for accuracy before submitting to direct supervisor and monitors financial expenditures as it pertains to youth financial benefits.
Participates in collaborative meetings with community partners to stay abreast of appropriate resources for HAY eligible youth.
Conducts regular individual conferences and unit meetings with staff to review issues conduct case reviews disseminate information complete staff evaluations etc.
Participates in and attends HAY Center events and PAL statewide and regional activities as required.
Bachelors degree in Education Psychology Sociology Social Work Criminal Justice or other Behavioral or Social Science
Experience:
Minimum two (2) years of full-time paid supervisory experience in a social service setting.
Minimum three (3) years of full- time paid experience working in a social service setting with transition age youth who have challenging life circumstances.
Training and experience using a personal computer with a variety of software such as Microsoft Outlook Microsoft Word Microsoft Excel PowerPoint and Case Management Software
Licensure:
Reliable transportation with a valid Texas drivers license and auto liability insurance.
Ability to become CPR certified.
Knowledge Skills & Abilities:
Knowledge of the transitional needs including life skills and linkages for youth aging out of the foster care system.
Knowledge of basic employee management techniques and an ability to handle stressful situations with tact and diplomacy are also necessary.
Must be responsible and self-disciplined and work independently.
Possess excellent verbal and written communication skills.
Self-motivated and possess excellent time management skills.
Must possess great organizational skills.
Have good public relations.
Effectively communicate with all parties involved with the process.
Establish and maintain effective working relationships with coworkers and business associates.
Ability to drive a 15-passenger van.
