What Youll Do
As an Placement Coordinator you will be responsible for ensuring a seamless admission experience for families guardians and people with disabilities as they transition to or begin to receive services from PPCH. Youll help support people receiving services in creating the lives they desire by facilitating successful partnerships and services. You will also act as liaison between our members receiving services Host Home Providers family members and team members to ensure the pursuit of the members goals and addition youll ensure that services and support are provided in a way that respects and acknowledges an individuals self-expression self-worth self-reliance and effective decision-making.
Some of what you will do is:
- Builds and maintains relationships with referral sources families guardians and people with disabilities looking to receive services in the community.
- Responds to inquiries comments and complaints promptly and professionally.
- Maintains exemplary services in all programs of responsibility through quality assurance measures and processes.
- Continually evaluates and assesses programs and makes recommendations regarding improvements to services supports and customer satisfaction.
- Ensures that training resources and work environment are provided to support individuals receiving services in pursuing their goals activities of interest and social interactions.
- Attends team meetings and ensures implementation of the person-centered service plan and applicable rights modifications for people receiving services.
- Works closely with the directors of PPCH programs to ensure a smooth transition into services.
- Evaluates and assesses the programs strengths and weaknesses to improve services and supports using a person-centered approach.
- Collaborates with and assists PPCH employees contracted providers and community stakeholders to carry out departmental and organizational goals and objectives.
- Ensures that the preferences of individuals receiving services from PPCH are considered and maintained according to standards.
- Conducts required assessments of individuals receiving services and ensures prompt follow-up when deficiencies or areas for improvement are found.
- Completes an inspection of the living environment for people receiving residential services from PPCH to ensure standards are met prior to any potential new placement.
- Supports internal moves and other logistical issues related to admissions. Coordinates movers arranges for storage facilities and coordinates with other agencies as needed.
- Connects individuals entering into residential services with other PPCH programs to ensure they receive comprehensive support and continuity of care.
- Models teaches and ensures respect for others including individuals in services staff providers guardians and loved ones and team members

What Youll Need
- High school diploma or equivalent is required (Bachelors degree is preferred)
- Successful completion of the required training provided by PPCH
- 2 years of work experience in the field of social services or human services serving children teens or adults with intellectual or developmental disabilities (I/DD) is required
- Strong computer skills with proficiency using Microsoft Office suite or similar software is required
- Excellent written and verbal communication skills including effective listening skills even when non-traditional communication is used is important
- Ability to lead influence direct and motivate team members families and contractors in order to provide quality services and onboard individuals seeking support quickly and collaboratively in their preferred setting is key
- Strong organizational skills effective and creative negotiation skills the ability to prioritize well think critically and problem-solve is necessary
- Must be dependable trustworthy and self-motivated with the ability to work independently with little or no direct supervision once trained
- Must have a valid drivers license and the ability to meet PPCHs driving requirements
Important - Physical Requirements and Working Conditions
- Work may be performed in a typical office environment in a private home setting or out in the community and may be performed in person over the phone or virtually
- Frequent lifting moving and/or carrying of approximately 30 40 pounds
- May be required to bend stoop squat or climb as it pertains to assessing an environment
- Work may include driving throughout the Denver metro area on a regular basis
