Are you passionate about making a difference in the lives of youth and families Hillcrest Youth Services is seeking a compassionate detail-oriented Placement Coordinator to join our dynamic team.

This position offers a starting pay of $19.00 per hour.

WHAT IS THIS POSITION RESPONSIBLE FOR

  1. Collect compile and disseminate necessary information on all referrals and admissions to Hillcrest staff when needed.
  2. Assist in developing initial treatment plan and goals upon admission to program.
  3. Orientate new residents to rules procedures and general residential programs and services.
  4. Coordinate visitation for residents with approved family members.
  5. Maintain communication with family members concerning resident appointments as required.
  6. Attend assessments with residents as needed.
  7. Oversee residents including approval of visits and phone contacts.
  8. Maintain approved contact lists.
  9. Oversee the needs of the youth while ensuring follow-up and information is communicated as appropriate.
  10. Coordination of after care services.
  11. Maintain communication with the referral agencies to assess community needs trends in placement and other relevant information.
  12. Assist referral source/agency in identifying other appropriate agencies or services if admission to Hillcrest is denied.
  13. Assist with coordination of treatment plan goals and residents needs with outside agencies and family members involved in the treatment of the resident.
  14. Maintain appropriate documentation.
  15. Provide family education.
  16. Weekly updates to placing agencies.
  17. Monthly program reports to placing agencies.
  18. Monitor residents grades on an ongoing basis and provide a printout monthly for the medical record if Educational Coordinator is out.
  19. Represent Hillcrest in an appropriate manner while at school or court.
  20. Maintain required twenty hours per year of ongoing training.
  21. Transport residents as needed.
  22. Obtain/maintain CPR First Aid.
  23. Facilitate other residential functions as assigned.



Qualifications

WHATS REQUIRED FOR THIS POSITION

  • Must hold a bachelors degree in social work or related human service field.
  • Must have knowledge of relevant clinical issues and demonstrate clinical expertise for populations served.
  • Must meet requirements as identifiedby State Licensing regulations.
