This position provides an exciting opportunity to practice patient-centered primary care within an integrated healthcare setting that combines medical and behavioral health one of only a limited number of organizations nationally designated as both a Certified Community Behavioral Health Clinic (CCBHC) and FQHC Liberty Resources offers physicians the opportunity to provide whole-person care in a highly collaborative environment focused on improving health outcomes for underserved successful candidate will work closely with primary care providers behavioral health clinicians nursing staff care managers and other healthcare professionals to deliver high-quality evidence-based care to diverse patient and BenefitsWe offer a highly competitive compensation package including:Base salary dependent on -based incentive opportunities.401(k) with employer match 457(b) plan and supplemental retirement weeks of vacation eight personal days and accrued sick CME time and -paid malpractice coverage licensure fees and professional site for NHSC and PSLF loan repayment patient panels and carved out administrative timeResponsibilitiesClinical PracticeProvide direct patient care in an outpatient primary care comprehensive medical services within your area of specialty including Family Medicine Internal Medicine or Med/ treat and manage acute and chronic medical preventive health screenings annual wellness visits and health maintenance and implement individualized treatment plans based on evidence-based with behavioral health team members to provide integrated care for patients with co-occurring medical and behavioral health in regular case conferences and interdisciplinary team meetings to coordinate care plans for complex care with specialists hospitals and community providers as accurate and timely electronic medical records in accordance with organizational to quality improvement initiatives and population health management care coordination activities that improve patient access engagement and health in backup telephonic on-call duty (Liberty Primary Care contracts with a service that takes first call).Demonstrate commitment to patient-centered compassionate and culturally responsive current with clinical best practices and continuing medical education or DO from an accredited medical Certified or Board Eligible in Family Medicine Internal Medicine or Med/ and unrestricted New York State medical license or ability to obtain one prior to registration or ability to obtain prior to of three years of clinical experience working in primary care settings interest in integrated care models and interdisciplinary to serving underserved and vulnerable in an FQHC community health center or similar setting communication teamwork and patient engagement Resources is committed to creating a diverse and inclusive environment and is proud to be an equal opportunity employer.
