JOB REQUIREMENTS
- Board: NP or PA certification
- Licenses: Active PA license required
- DEA: DEA required
- Certifications: ACLS BLS
- All required documentation must be submitted no later than 5 business days prior to start
- Provider questionnaire required at time of submission
- NPDB self-query required at time of presentation within 6 months
- Full disclosure on all malpractice required
- If the role includes specific coverage dates your submission must clearly state exact dates the provider can work
- Submissions without rates will be rejected immediately
- Required Submission Packet: CV Coversheet Questionnaire NPDB (pulled within the last 6 months)
- Provide clinical support within the Interventional/Electrophysiology department
- Manage patient care for ages 18-100 with an average volume of 18 patients per day
- Perform post-operative assessments for patients undergoing procedures such as Pacemaker/ICD implants and ablations
- Collaborate with the assigned clinical team to ensure high-quality patient outcomes
- Maintain accurate documentation within the EPIC EMR system
