Physical Therapy Homecare Experience

We are seeking a dedicated Physical Therapy professional with homecare experience to join our team at University Home Care this role you will provide high-quality patient-centered physical therapy services in the comfort of patients homes helping them improve mobility manage pain and regain independence.

• Deliver physical therapy treatments to patients in their homes according to prescribed plans of care.
• Evaluate patient progress and adjust treatment plans as necessary.
• Maintain accurate and timely documentation of patient care and treatment outcomes.
• Communicate effectively with patients families and healthcare team members.
• Ensure compliance with all company policies professional standards and regulatory requirements.

• Valid license as a Physical Therapist.
• Previous homecare experience preferred.
• Strong assessment and clinical skills.
• Excellent communication and interpersonal skills.
• Reliable transportation and ability to travel within the Southeast MI Regional area.
