Required:

• Associate degree or higher
• 1 years of experience in ANY of the following:
  • Home Health Care
  • Home Health
• 1 years of experience in ANY of the following:
  • Electronic Medical Record (EMR)
  • Practice Partner EMR
  • EHR Implementation
• Valid Regular Driver License (DL) in the state of Pennsylvania (PA)
• Valid Physical Therapy Assistant in the state of Pennsylvania (PA)
• Are you okay with the required travel for this position (yes)

Join our clients care team as a Home Health Physical Therapy Assistant providing compassionate support to patients in their homes. You will help manage and deliver patient care plans while working closely with a team dedicated to outstanding care.

Key Responsibilities:

Collaborate to provide management and delivery of patient care plans
Support patients in Lebanon PA and surrounding areas
Meet critical deadlines for charting
Self-manage time and schedule effectively Travel
Ability to spend approximately 20% of the day driving to/from patient locations

Schedule: Monday-Friday 8am-5pm *no on-call or weekends
