Physical Therapy Assistant

We seek a highly skilled and experienced Registered Nurse with specialties in Medical/Surgical ICU ER Psychiatric Labor & Delivery PACU Operating Room. The Registered Nurse will be responsible for providing skilled nursing care case management and hospice care to individuals facing advancing age frailty and serious illness. Daily tasks include patient assessments medication administration treatment planning and collaborating with the interdisciplinary team. Critical to this role is strong communication and collaboration with patients providers and other members of our team. At least 2 years of experience and exceptional skill is required.

• Provide skilled nursing care to patients in various specialties including Medical/Surgical ICU ER Psychiatric Labor & Delivery PACU and Operating Room.
• Conduct patient assessments and monitor patient status regularly.
• Administer medications and treatments as prescribed and required.
• Develop and implement individualized treatment plans.
• Collaborate effectively with interdisciplinary teams including physicians therapists and other healthcare professionals.
• Manage case documentation and ensure compliance with healthcare regulations.
• Provide hospice care and support to patients and their families.
• Communicate clearly and compassionately with patients and healthcare team members.

• Active Registered Nurse license in the state of California.
• Minimum of two years experience in hospital home health or hospice care settings.
• Clinical assessment medication administration and treatment planning skills.
• Bachelors degree in Nursing or related field.
• Drivers license car insurance and automobile for field work.
• Strong interpersonal and communication skills.

• Experience with SNF (Skilled Nursing Facility) settings.
• Familiarity with NetHealth (Rehab Optima) software.

• Must maintain an active California nursing license.
• Ability to work in a fast-paced healthcare environment.
• Collaboration with interdisciplinary teams is critical.

• Work schedule and shifts to be determined based on facility needs and patient care requirements.
