We are currently seeking licensed Physical Therapists to provide home health services across Oahu.

Responsibilities:
- Conduct skilled therapy visits in patients homes
- Perform PT evaluations and follow-up assessments
- Develop fall prevention and safety strategies
- Implement mobility balance and strengthening programs
- Educate patients and family caregivers
- Complete accurate Medicare-compliant documentation
Qualifications:
- Current Hawaii license as PT
- Previous home health experience is a plus
- 1 year clinical experience (home health preferred)
- Strong documentation and communication skills
- Drivers license reliable transportation
- Excellent clinical communication and organizational skills
