Physical Therapist Skilled Nursing Facility

Location: Phoenix AZ

Duration: 13 Weeks Contract

Shift: Day Shift

Hours: 8 Hour Shifts 32 Hours/Week Guaranteed

We are seeking a compassionate and motivated Physical Therapist for a Skilled Nursing Facility setting. This opportunity offers the chance to work closely with patients to improve mobility strength and overall quality of life.

Responsibilities:
Evaluate patients and develop individualized physical therapy treatment plans
Provide therapeutic exercises mobility training and rehabilitation services to patients recovering from injury illness or surgery
Monitor patient progress and adjust treatment plans as needed to achieve rehabilitation goals
Educate patients and family members on therapy programs exercises and injury prevention techniques
Collaborate with physicians nurses and interdisciplinary healthcare teams to coordinate patient care
Maintain accurate and timely documentation of patient treatments progress notes and discharge planning
Assist patients in improving strength balance flexibility mobility and functional independence
Ensure compliance with facility policies patient safety standards and healthcare regulations
Participate in care planning meetings and contribute to quality patient outcomes

Requirements:
Active Physical Therapist (PT) license in Arizona or compact eligibility required
Graduate of an accredited Physical Therapy program required
Strong communication patient care and clinical assessment skills required
Ability to work independently and collaboratively within a multidisciplinary healthcare team required
Excellent organizational and documentation skills required

Preferred:
Current CPR/BLS certification preferred
Minimum 1 year recent Physical Therapist experience preferred
Skilled Nursing Facility (SNF) or rehabilitation experience preferred
