Job Summary
We are seeking a skilled and compassionate Physical Therapist to join a Skilled Nursing Facility in Merced California. This role involves working with a geriatric and post-acute population helping patients recover from surgery illness or injury while improving mobility and overall function. You will be responsible for evaluating patients developing individualized treatment plans and guiding them toward greater independence and quality of life. Job Responsibilities:
Evaluate diagnose and treat patients with physical impairments in a Skilled Nursing Facility setting
Develop and implement individualized treatment plans based on patient needs and clinical findings
Supervise and collaborate with Physical Therapist Assistants to ensure effective care delivery
Monitor patient progress and modify treatment plans as needed
Educate patients and caregivers on mobility safety and home exercise programs
Maintain accurate and timely documentation in compliance with facility and regulatory standards

Requirements
Active state license as a Physical Therapist in California
Current BLS/CPR certification
Minimum of 1 year of recent PT experience preferred ideally in a SNF or geriatric setting
Strong clinical reasoning and leadership skills
Ability to work collaboratively within an interdisciplinary team

Required Skills:
Job Requirements: Active state license as a Physical Therapist in $ Province Current BLS/CPR certification preferred 01 year of experience welcome (new graduates encouraged to apply) Experience or clinical exposure in SNF rehabilitation or geriatric care is a plus Strong clinical reasoning and patient care skills Ability to work collaboratively within an interdisciplinary team
