About the Role
We are seeking a Physical Therapist for a 13-week contract opportunity in Santa Rosa California. This role supports patients in a skilled nursing facility setting and focuses on helping residents improve mobility strength balance and functional independence through quality rehabilitation care.
Responsibilities
Evaluate patients and develop individualized physical therapy treatment plans
Provide therapeutic exercises mobility training gait training and functional rehabilitation
Monitor patient progress and adjust treatment plans based on clinical needs
Educate patients caregivers and facility staff on safety mobility and therapy goals
Document therapy services accurately and collaborate with the interdisciplinary care team
Requirements
Active California Physical Therapist License Required
Skilled Nursing Facility experience preferred
Strong patient assessment treatment planning and communication skills required
Ability to manage a varied rehab caseload in a post-acute setting
Must be available for varied shifts and 13-week assignment
