Were looking for a full-time Physical Therapist to serve patients in Marin County. This individual would be based out of their home while traveling to and from patient homes for clinical care.

Key Responsibilities

• Evaluate patients functional status muscle strength mobility gait and ROM to develop and direct individualized treatment plans
• Instruct patients families and caregivers on therapeutic appliances PT goals and relevant care phases
• Monitor and report patient progress and condition changes to the physician
• Prepare and submit clinical progress summaries and participate in discharge planning
• Identify needs for additional home health services and refer accordingly
• Supervise Physical Therapy Assistants and when PT is the sole skilled service evaluate home health aide care every two weeks
• Provide consultation staff in-services and participate in peer review as needed

Qualifications

• Bachelors or Masters degree in Physical Therapy from an accredited program
• Active state PT license
• Minimum 2 years of PT experience (home health/community experience preferred)
• Strong verbal and written communication skills
• Current CPR certification
• Valid drivers license insured vehicle in good working order
