About the Role

We are seeking a Physical Therapist for a 13-week assignment in a skilled nursing and long-term care setting in San Antonio Texas. This role is ideal for a therapy professional who is confident in evaluating patients developing treatment plans and supporting functional recovery for residents.

Responsibilities

• Evaluate patients and develop individualized therapy plans

• Provide skilled physical therapy services in a SNF/LTC setting

• Support mobility strength balance and functional improvement

• Document treatment progress and patient response accurately

• Collaborate with the care team to support patient outcomes

• Maintain safe therapy practices and follow facility standards

Requirements

• Active Texas Physical Therapist license required

• Minimum 1 year of Physical Therapist experience required

• Skilled Nursing Facility or Long-Term Care experience preferred

• Strong clinical assessment and patient care skills

• Ability to work independently and as part of a care team
