Responsibilities:
Evaluate mobility strength balance gait and rehabilitation needs.
Establish measurable goals and individualized treatment plans.
Deliver appropriate therapeutic interventions and monitor patient outcomes.
Modify treatment plans as patients progress.
Educate patients and caregivers on safe mobility and continued recovery.
Coordinate care with rehabilitation and nursing colleagues.
Complete timely clinical documentation and support discharge planning.

Qualifications:
Graduate of an accredited physical therapy program.
Current California PT license or eligibility to obtain licensure before starting.
Strong clinical reasoning communication and documentation skills.
Commitment to collaborative patient-centered rehabilitation.
