Kace Premier Medical Talent is recruiting a Physical Therapist for a skilled nursing rehabilitation opportunity in West Reading Pennsylvania. Support patients working toward improved mobility safer movement and greater independence.

Responsibilities:
Perform physical therapy evaluations and identify functional limitations.
Establish measurable goals and individualized treatment plans.
Provide therapeutic exercise gait training balance training and functional mobility interventions.
Monitor outcomes and adjust treatment as appropriate.
Educate patients and caregivers on safe movement and prescribed exercises.
Coordinate with rehabilitation and nursing colleagues.
Complete accurate clinical documentation and support discharge planning.

Qualifications:
Graduate of an accredited physical therapy program.
Current Pennsylvania PT license or eligibility to obtain licensure before starting.
Strong assessment treatment planning and documentation skills.
Ability to work effectively with an interdisciplinary team.
