We are seeking a Physical Therapist for a 12-week contract opportunity in Sacramento California. This role supports a skilled nursing and post-acute rehabilitation facility with an experienced rehab team and a varied caseload in a fast-paced team-focused environment.
Responsibilities
Evaluate patients and develop individualized physical therapy treatment plans
Provide therapy services to support mobility strength balance and functional independence
Manage a varied caseload and prioritize patient care needs effectively
Document therapy services accurately using PointClickCare
Collaborate with the rehab and nursing teams to support patient recovery and outcomes
Requirements
Active California Physical Therapist License Required
SNF or post-acute rehabilitation experience preferred
Strong clinical reasoning communication and documentation skills required
Must be adaptable team-oriented and comfortable in a fast-paced environment
Must be available for Monday Friday morning shift schedule
