Job Summary

Job Title: Physical Therapist

Salary Range: $45-$48 per hour

Schedule: 35 hours per week Monday-Friday no weekend rotation

Facility: 120-bed skilled nursing facility (SNF)

Job Summary:

We are seeking a highly skilled and dedicated Physical Therapist to join our team at our 120-bed skilled nursing facility. The ideal candidate will have a passion for helping others and a strong background in physical therapy. As a Physical Therapist you will work with patients to help them regain mobility manage pain and improve overall physical function.

Key Responsibilities:

- Evaluate patients medical history and perform physical assessments to determine appropriate treatment plans

- Develop and implement individualized treatment plans based on patients needs and goals

- Utilize a variety of techniques and modalities to improve patients mobility strength and range of motion

- Educate patients and their families on exercises and techniques to continue at home

- Monitor and document patients progress and adjust treatment plans as needed

- Collaborate with other healthcare professionals such as doctors and occupational therapists to provide comprehensive care for patients

- Maintain accurate and up-to-date patient records

- Adhere to all state and federal regulations and ethical standards

Qualifications:

- Bachelors Masters or Doctorate degree in Physical Therapy from an accredited program

- Current state license to practice Physical Therapy

- Minimum of 2 years of experience in a skilled nursing facility or similar setting

- Strong knowledge of physical therapy principles techniques and modalities

- Excellent communication and interpersonal skills

- Ability to work independently and as part of a team

- Compassionate and patient-centered approach to care

- Basic computer skills and experience with electronic medical records
