What Youll Do

• Perform comprehensive physical therapy evaluations and establish individualized treatment plans
• Deliver evidence-based interventions focused on mobility balance strength pain management and functional independence
• Monitor patient progress and adjust treatment strategies to maximize outcomes
• Collaborate with physicians nursing staff families and interdisciplinary care teams
• Provide supervision and direction to Physical Therapy Assistants in accordance with state regulations
• Complete timely accurate and compliant clinical documentation
• Educate patients and caregivers on safe mobility techniques and discharge recommendations
• Participate in care conferences team meetings and quality improvement initiatives
• Promote a positive patient experience through compassionate and patient-centered care

Qualifications

• Graduate of an accredited Physical Therapy program
• Current Ohio Physical Therapist license in good standing
• Skilled nursing post-acute or geriatric rehabilitation experience preferred
• Excellent communication and clinical reasoning skills
• Ability to build strong relationships with patients families and healthcare professionals
