Responsibilities
Evaluate residents and develop individualized treatment plans
Provide skilled physical therapy services to improve mobility strength and independence
Supervise PTAs and collaborate with the interdisciplinary care team
Participate in care planning and discharge preparation
Educate residents families and caregivers
Maintain accurate and timely clinical documentation
Qualifications
Current California Physical Therapist (PT) license
Strong clinical and communication skills
Commitment to resident-centered care
Ability to work effectively within a multidisciplinary team
