Concept Rehab is looking for a Physical Therapist (PT) to join our team on a PRN basis in Linden Michigan.

• Evaluate patients and develop individualized treatment plans
• Provide skilled physical therapy services to help patients improve mobility strength balance and independence
• Document patient progress and treatment according to professional standards
• Collaborate with nursing therapy and other members of the care team
• Provide compassionate patient-centered care

• Current Physical Therapist (PT) license in the state of Michigan
• Graduate of an accredited Physical Therapy program
• Strong communication and organizational skills
• Reliable and compassionate approach to patient care
• Previous experience in a skilled nursing or post-acute setting is helpful but not required
