This position supports patients throughout the West Houston market including areas such as Houston Heights and Jersey Village; Katy; Sugar Land and Missouri City; Richmond and Rosenberg; and surrounding communities. Specific patient assignments will be determined based on clinician location and business needs.

As a Physical Therapist youll contribute to our success in the following ways:

• Evaluate patients and develop individualized treatment plans.

• Provide physical therapy services in the home setting.

• Monitor patient progress and adjust care plans as needed.

• Collaborate with physicians caregivers and the interdisciplinary care team.

• Complete timely and accurate clinical documentation.

• Supervise therapy assistants and support staff as appropriate.

What is Required

• Graduate of an American Physical Therapy Association (APTA) approved physical therapy program

• Current unrestricted PT license valid for the state of work

• Minimum one (1) year of experience in a healthcare facility

• Must possess a valid drivers license maintain an acceptable driving record OR have reliable transportation to travel between patient homes and assigned work locations.

• Ability to frequently lift push pull and support up to 50 pounds including positioning or transferring patients and moving equipment
