• Evaluate and treat patients in a post-acute/skilled nursing setting
• Develop individualized treatment plans focused on improving mobility strength balance and independence
• Collaborate with Occupational Therapists Speech-Language Pathologists nursing staff physicians and families
• Complete accurate timely documentation
• Educate patients and caregivers on treatment plans and home exercise programs
• Help patients achieve meaningful functional outcomes

• Licensed Physical Therapist (PT) in the State of Ohio
