• Provide skilled physical therapy services in patients homes.
• Perform comprehensive initial evaluations reassessments and fall risk assessments.
• Develop individualized treatment plans to improve strength mobility balance and functional independence.
• Educate patients and caregivers on home exercise programs and injury prevention.
• Collaborate with nurses physicians and the interdisciplinary care team to ensure quality patient outcomes.
• Complete accurate timely and compliant clinical documentation.

• Current Hawaiʻi Physical Therapist (PT) license in good standing.
• Minimum of one year of clinical experience preferred.
• Home Health experience is a plus but not required.
• Strong clinical judgment communication and documentation skills.
• Valid drivers license reliable vehicle and current auto insurance.
• Ability to travel throughout Oʻahu to provide in-home patient care.
• Passion for serving seniors with compassion and professionalism.
