Location: San Mateo County CA
Pay Rate: $57.00/hr $67.00/hr
Pay Per Day: $456 $536/day based on 8 hours ( Pay cycle is bi weekly)
Pay per Week : $2010.00/week - $2600.00/week
Contract: 13 Weeks
Schedule: Day Shift 8:00 AM 5:00 PM
Total Working Hours: 40 Hours/Week
Experience: Minimum 1 Year Home Health Experience
About the Role
We are seeking an experienced Physical Therapist for a 13-week Home Health contract opportunity in San Mateo County CA. This role focuses on restoring patient function preventing or minimizing disability and helping patients achieve their maximum level of independence through medically prescribed therapy programs.
Responsibilities
- Evaluate and treat Home Health patients using medically prescribed physical therapy plans.
- Perform initial admissions evaluations skilled treatments and follow-up therapy visits.
- Document clinical findings patient progress skilled interventions goals and discharge plans.
- Notify the Clinical Team Manager of OASIS scores within required timelines.
- Travel within the assigned territory and neighboring cities based on patient care needs.
Requirements
- Active California Physical Therapist License Required.
- BLS/BCLS Certification Required.
- Minimum 1 Year Of Home Health Experience Required.
- Homecare Homebase / HCHB Experience Preferred.
- Ability To Work Flexible Day Schedule Based On Patient Availability.
