Pay Rate: $55.00/hr $62.00/hr
Pay Per Day: $440 $496/day based on 8 hours ( Payment cycle : bi- weekly)
Pay Per Week : $ 1980.00/week - $2232.00/week
Contract: 13 Weeks
Schedule: Day Shift
Total Working Hours: 36 Hours/Week
Experience: Minimum 1 Year
About the Role
We are seeking a qualified Physical Therapist for a 13-week Home Health contract opportunity covering the San Jose and Santa Clara County territory. This role includes providing patient care in home health settings completing visits and documentation and meeting weekly productivity expectations.
Responsibilities
- Provide Physical Therapy services to patients in a Home Health setting.
- Complete evaluations re-certifications routine visits discharges and follow-up visits as assigned.
- Maintain accurate clinical documentation and complete required visit records.
- Travel within assigned territory and occasionally outside the area based on patient care needs.
- Communicate professionally with patients families physicians and care team members.
Requirements
- Active California Physical Therapist License Required.
- BLS/BCLS Certification Required.
- Valid Drivers License Auto Insurance And Auto Registration Required.
- Annual TB MMR Varicella Hep B TDap Flu Drug Screen And Physical Required.
- Home Health Experience Preferred; Must Be Willing To Work Weekends Or Travel Outside Territory If Needed.
