Shift: Days Shift Length: 8 Hours Guaranteed Hours: 32 Hours/Week

Navitas Healthcare LLC is seeking Physical Therapist Skilled Nursing Facility for an exciting Travel or Local job in Phoenix AZ.

• Evaluate patients and develop individualized physical therapy treatment plans
• Provide therapeutic exercises mobility training and rehabilitation services to patients recovering from injury illness or surgery
• Monitor patient progress and adjust treatment plans as needed to achieve rehabilitation goals
• Educate patients and family members on therapy programs exercises and injury prevention techniques
• Collaborate with physicians nurses and interdisciplinary healthcare teams to coordinate patient care
• Maintain accurate and timely documentation of patient treatments progress notes and discharge planning
• Assist patients in improving strength balance flexibility mobility and functional independence
• Ensure compliance with facility policies patient safety standards and healthcare regulations
• Participate in care planning meetings and contribute to quality patient outcomes

• Active Physical Therapist (PT) license in the state of Arizona or compact eligibility
• Graduate of an accredited Physical Therapy program
• Current CPR/BLS certification preferred
• Minimum of 1 year of recent Physical Therapist experience preferred
• Skilled Nursing Facility (SNF) or rehabilitation experiencerequired
• Strong communication patient care and clinical assessment skills
