BAYADA Home Health Careis currently seeking an experiencedPhysical Therapist PTfor a full-timeopportunity performing home healthvisits for our Atlantic County Visits office. This office services adult and geriatric clients on a per visit basis in territories throughoutAtlantic County.

As a home care Physical Therapist you will be an integral member of a multi-disciplinary health care team that provides rehabilitative care and skilled nursing to clients affording them the opportunity to receive the medical care required to remain at home.

• Perform home visits in designated geographic territories
• Complete comprehensive physical therapy evaluations
• Assess mobility strength balance gait transfers endurance and safety risks
• Develop individualized treatment plans to improve function and independence
• Provide therapeutic exercise gait training balance training and other evidence-based interventions
• Collaborate with nurses and other clinicians as part of a multidisciplinary care team
• Educate patients and caregivers on exercises safety and disease management
• Document patient care using a user-friendly electronic medical record system

Qualificationsfor a PT - Physical Therapist include:

• A current NJPhysical Therapist - PT license.
• A minimum of one year of recent work experience as a Physical Therapist - PT.
• Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association as indicated by school transcript or diploma.
• Ability to work independently and manage time effectively.
• Strong interpersonal organizational and problem solving skills.
• Solid computer skills; prior experience with electronic medical records (EMR) preferred.

Additional Information:

Base Pay depending upon qualifications status and subject to negotiation:

$64.00 - $72.00 per point ($99840 - $112320 annual earning potential)
