Position Summary

We are seeking a compassionate and motivated Physical Therapist to join our this role you will evaluate and treat patients with a variety of conditions while developing individualized treatment plans designed to help each patient achieve their goals.

Key Responsibilities

• Evaluate patients and develop individualized plans of care based on clinical findings and patient goals
• Provide evidence-based physical therapy treatments and interventions
• Monitor patient progress and adjust treatment plans as appropriate
• Educate patients and caregivers regarding treatment plans exercises and strategies to support continued progress
• Maintain accurate and timely clinical documentation
• Communicate and collaborate with physicians healthcare providers and members of the clinical team
• Provide a positive patient-centered experience throughout the course of care
• Follow applicable professional ethical regulatory and clinic standards

Qualifications

• Graduate of an accredited Physical Therapy program
• Current Connecticut Physical Therapist license or eligibility for licensure
• Strong communication and interpersonal skills
• Commitment to providing high-quality patient-centered care
• Ability to work collaboratively as part of a clinical team
• Desire to continue learning and developing professionally
