DW Sisters LLC is placing a Travel Physical Therapist with Trinity Health in Muskegon Michigan. This position requires Skilled Nursing Facility (SNF) experience and includes some holiday and weekend availability (one weekend per month).
Shift Details: Muskegon MI 49442 8:00 AM 5:00 PM MondayFriday 40 Hours/Week Department: Physical Therapy Some holidays and one weekend per month required
Responsibilities:
- Perform initial and ongoing assessments of patient condition
- Establish revise and evaluate individualized plans of care involving patient and family
- Perform therapy interventions using standard PT techniques appropriate to the patients condition
- Oversee activities of PT assistants students and other support personnel
- Provide PT services in a skilled nursing facility (SNF) environment
- Requirements: Active Physical Therapist (PT) license Michigan licensure required or must be obtainable SNF (Skilled Nursing Facility) experience required 2 years of physical therapy experience Availability for some holidays Availability for one weekend per month Travel PT experience preferred
