Location: Missoula Montana
Pay Rate: $56/hr
Contract: 13 weeks
Schedule: Monday Friday 8:00 AM 5:00 PM
Guaranteed Hours: 40 hours per week
Experience: Minimum 2 years required
About the Role
We are seeking an experienced Home Health Physical Therapist for a 13-week rapid response contract opportunity in Missoula Montana. This role provides physical therapy evaluations rehabilitation interventions patient education and care coordination in the home setting across assigned service areas.
Responsibilities
Evaluate patients in the home setting and develop individualized physical therapy plans of care
Provide direct therapy interventions based on physician orders and patient needs
Reassess patient progress and modify treatment plans frequency and duration as appropriate
Coordinate care with physicians schedulers PTAs and interdisciplinary team members
Document skilled services accurately using Homecare Homebase and support patient/family education
Requirements
Active Montana Physical Therapist License Required
Minimum 2 years of Physical Therapist experience preferred/required
BLS/BCLS Certification Required
Home Health experience and Homecare Homebase experience preferred
Valid drivers license auto insurance own vehicle and availability for on-call weekend and holiday coverage required
