McKinley Park Care Center is seeking a compassionate and dedicated Per-Diem Physical Therapist (PT) to join our dynamic rehabilitation team in Sacramento CA. If youre passionate about helping patients regain mobility improve function and achieve greater independence we want to hear from you.

As a Physical Therapist youll work alongside an experienced interdisciplinary team providing exceptional rehabilitative care to residents recovering from illness injury surgery and other medical conditions in a skilled nursing environment.

The Physical Therapist evaluates residents develops individualized treatment plans provides therapeutic interventions and collaborates with physicians nursing staff families and rehabilitation team members to achieve optimal functional outcomes. This position also supports staff education quality improvement initiatives and regulatory compliance.

• Evaluate residents physical therapy needs functional abilities and rehabilitation potential.
• Develop implement and revise individualized treatment plans based on physician orders and clinical findings.
• Provide evidence-based interventions to improve mobility strength balance endurance coordination and functional independence.
• Establish realistic goals and monitor progress toward achievement.
• Educate residents and families on treatment plans home exercise programs fall prevention and safety techniques.
• Recommend appropriate adaptive equipment and assistive devices.

• Communicate resident progress and treatment recommendations to physicians nursing staff families and interdisciplinary team members.
• Participate in care plan meetings case conferences rehabilitation meetings and discharge planning discussions.
• Collaborate with rehabilitation staff to maximize resident outcomes.
• Support restorative nursing and rehabilitation programs.
