Platinum Home Health Services is seeking a compassionate motivated Physical Therapist (PT) to join our growing home health team. The ideal candidate is committed to providing high-quality patient-centered care while helping patients improve mobility function and overall quality of life in the comfort of their own homes.

• Perform comprehensive physical therapy evaluations and develop individualized plans of care
• Provide skilled physical therapy services in accordance with physician orders and agency policies
• Assess patient mobility strength balance and functional limitations
• Implement therapeutic exercises gait training and pain-management techniques
• Educate patients and caregivers on treatment plans safety and home exercise programs
• Document evaluations progress notes and discharge summaries accurately and timely
• Communicate effectively with physicians case managers and interdisciplinary team members
• Follow all state federal and Medicare home health regulations

• Current Physical Therapist license in the State of Ohio
• Graduate of an accredited Physical Therapy program
• Home health experience preferred but not required
• Strong clinical communication and organizational skills
• Ability to work independently and manage a caseload
• Valid drivers license reliable transportation and auto insurance
