Responsibilities
Monday-Friday: 8:00 AM - 4:30 PM
- Provide physical therapy evaluations and treatments for inpatient rehabilitation patients.
- Develop and implement individualized treatment plans based on patient needs and clinical goals.
- Assess patients mobility strength balance functional ability and rehabilitation needs.
- Provide therapeutic interventions to improve mobility independence and activities of daily living.
- Monitor and document patient progress accurately in Epic.
- Collaborate with physicians nurses therapists and other members of the interdisciplinary care team.
- Assist with discharge planning and recommendations for continued care.
- Maintain compliance with facility policies clinical standards and patient safety requirements.
- Provide care for a diverse inpatient population including patients who may require COVID-related precautions.
- Minimum 2 years of Physical Therapy experience.
- Current Pennsylvania PT license or eligibility through a compact license.
- Epic experience required.
- Current professional credentials as required by the facility.
- Strong clinical assessment communication documentation and patient-care skills.
- Ability to work effectively in a multidisciplinary environment.
