Physical Therapist

We are seeking a dedicated Physical Therapist to provide rehabilitation services in inpatient and long-term care settings. The ideal candidate will evaluate patients develop individualized treatment plans and deliver therapy services focused on rehabilitation and functional improvement for adult patients. This position may also include outpatient rehabilitation services and requires flexibility to support departmental operations including weekends and holidays.

• Perform comprehensive patient assessments and develop individualized treatment plans.
• Provide physical therapy services for adult patients in inpatient skilled nursing rehabilitation and outpatient settings.
• Implement therapeutic interventions to improve mobility strength balance and overall functional independence.
• Monitor patient progress and modify treatment plans as appropriate.
• Maintain accurate and timely documentation in the electronic medical record system.
• Collaborate with interdisciplinary healthcare teams to coordinate patient care.
• Educate patients and families regarding therapy goals exercises and discharge planning.
• Maintain compliance with facility policies professional standards and state regulations.
• Provide coverage as needed based on departmental scheduling and operational needs.

• Variable schedule with departmental coverage seven days per week.
• Five-day workweek with possible weekend rotation.
• Potential schedules may include Sunday Thursday or Tuesday Saturday shifts.
• Must be available to work weekends and holidays as required.

• Minimum of 6 months of experience working with adult patients in at least one of the following settings:
  • Acute Care
  • Skilled Nursing Facility (SNF)
  • Rehabilitation Hospital
  • Outpatient Rehabilitation
• Experience in inpatient and long-term care rehabilitation preferred.
• Strong communication and patient care skills.
• Ability to work independently and collaboratively within a multidisciplinary team.

• Active Hawaii Physical Therapy license required by assignment start date.
• Candidates without an active Hawaii license at the time of application must apply upon offer acceptance.
• Current BLS/BCLS certification required.

• Assignment length: 13 weeks
• EMR system utilized: Meditech
• Holiday coverage required.
• Candidates must arrive prior to assignment start to complete required employee health clearance and onboarding requirements.
