We are seeking a compassionate and motivated Physical Therapist to join a growing Home Health and Assisted Living team serving the Parker Castle Rock and Castle Pines communities. This role offers the opportunity to provide individualized one-on-one patient care while helping patients regain mobility independence and overall quality of life in the comfort of their home or assisted living environment.
As a Physical Therapist you will evaluate patient needs develop personalized treatment plans and implement evidence-based interventions focused on improving strength balance flexibility coordination and functional mobility. - Evaluate patients and identify individualized goals and treatment needs
- Develop personalized care plans using evidence-based treatment techniques and interventions
- Provide skilled therapy services focused on improving mobility strength balance flexibility coordination and overall function
- Help patients restore movement reduce pain and maximize independence and quality of life
- Educate patients and caregivers on exercises wellness strategies safety techniques and injury prevention
- Monitor patient progress and modify treatment plans as needed to achieve optimal outcomes
- Collaborate with physicians nurses caregivers and interdisciplinary healthcare professionals to coordinate care
- Maintain timely and accurate documentation of patient evaluations treatments and progress notes

- Graduate of an accredited Physical Therapy program with a BS MS MPT or DPT degree
- Current Colorado Physical Therapist license
- Strong clinical assessment and treatment skills
- Effective written verbal and interpersonal communication skills
- Ability to work independently while collaborating within a multidisciplinary team
- Minimum of one year of Physical Therapy experience
- Home Health or Assisted Living experience preferred but not required
